Opening your online patient forms is an easy process.

Once your patient has filled out the form and clicked “Submit”, you will receive an email with a locked PDF.

Our team sent you a Password during your initial setup.

Download the PDF to your computer, and enter the password we sent to open the PDF.

This may be a pain, but it’s part of the HIPAA process.
